Job DescriptionExciting New Senior Living Community (Independent Living, Assisted Living and Memory Care) Opening Soon! The LPN - Care Partner provides direct nursing care to residents in Assisted Living and/or Memory care and delegates and supervises the care provided by Quality-of-Life Specialists (Caregivers) and Medication Aides.
The LPN - Care Partner administers medications, performs treatments and procedures, obtains specimens as needed, obtains physician's orders, and completes documentation as required.
The LPN will report to the Director of Health Services.
The anticipated start date is the end of May 2023.

Responsibilities:
Provide daily supervision and leadership to assigned staff and other designated personnel in meeting the resident's needs.
Demonstrate good decision-making skills.
Provide written evaluation and oral counseling of assigned staff.
Assist in finding replacements for call-ins.
Assist residents with Activities of Daily Living (ADL), family/social activities and actively promote independent lifestyle of residents.
Provide nursing care according to physician's orders and in conformance with recognized nursing practice, established standards, and administrative policies.
Participate in the admission of new residents.
Assure resident safety and treat all residents with respect and dignity.
Maintain confidentiality of resident, observe, and abide by all regulations to ensure that resident personal health information is protected.
Assess the resident, recognize any behavioral or medical changes, determine the appropriate nursing intervention, notify attending physician and responsible party in the event of a change in the resident's condition, and provide continuous monitoring and accurate documentation of resident status as per policy.
Accurately administer prescribed medications, supervise the continuity of nursing care, respond to changes in medical condition, medications or other circumstances requiring special care needs and perform first aid as required.
Respond to resident call system promptly and courteously.
Provide daily report to the Director of Health Services concerning community conditions, personnel performance, resident conditions, and needs for nursing supplies.
Make rounds with physician(s), assist in examinations and/or treatment of residents and carry out orders.
Orient residents and staff to emergency procedures and respond to all emergency situations.
Ensure that all resident records are accurate, complete and all mandatory forms are on file.
Through familiarity and application of community policies and procedures, assure that nursing care is practiced in full accordance with regulatory requirements, community quality standards, and resident rights.
Monitor resident environment to ensure all safety and sanitary regulations and quality standards are met or exceeded.
May assist with development and presentation of in-service programs.
Supports creating a culture where resident needs are exceeded including the wellbeing and vibrancy of residents.
Here are a few of the qualifications we need you to have:
Graduate of an accredited Nursing School (LPN) Current and valid nursing license in the State of New Jersey First Aid and/or CPR certification by qualified training Minimum of (1) years' experience in Nursing Experience in assisted living and memory care required Willingness to be available for any/all emergencies regarding the community Many roles in the community may require that we ask about your vaccination status.
This could include MMR, chicken pox, hepatitis, flu, and COVID-19 vaccine status.

The check will bounce and the victim is left responsible.
